A systematic review has assessed nutritional status and supplementation across vitamin complexes and iron in this population [2].
Maca holds the same grade on 5 studies and 300 participants, studied principally for vasomotor and mood symptoms rather than for body composition. Its inclusion reflects the pattern of the whole field: supplement research in menopause concentrates on symptom relief and on skeletal endpoints rather than on weight. A randomized placebo-controlled trial of resveratrol, for instance, reported improved bone mineral density in postmenopausal women [3], an outcome of genuine importance that says nothing about adiposity. Broader phytocomplex work has likewise measured well-being in climacteric women rather than weight [4].
The best-documented intervention for the metabolic changes of this period is not nutritional. A three-year controlled study of exercise in early postmenopausal women examined effects across menopausal risk factors including bone density and metabolic markers [5]. Resistance training addresses the lean-mass loss accompanying the transition, and lean mass is the principal determinant of the resting energy expenditure against which weight is maintained.
Three limits apply. The trials in this set are small, 300 to 500 participants per ingredient, and short relative to a transition measured in years. Menopausal weight change is confounded with chronological aging, and few designs separate the two. And symptom overlap is substantial: hot flashes, sleep disruption, mood change, and decreasing bone density accompany the weight change and are the endpoints most trials actually measure. Hormone therapy and its alternatives are clinical decisions outside the scope of a supplement grading, and vitamin D dosing should follow measured status.
